Please make sure that all of the following is completed for ALL Players to enable me to complete Registrations MAKE SURE THE MEDICAL DETAILS ARE COMPLETED IN THE BACK OF THE PASSPORT !!!!!!!!! Please complete a Players registration card which requires 3 signatures -Player, Parent or Guardian & Manager/ Coach Please complete players passport - Remember it must have the Players signature Players Passport - NEW PAGE for under 11 Existing Player - YOU CANNOT USE EXISTING PHOTOS I require 2 new upto date photos { if no new photos are supplied then the registration will be rejected untill new photos are supplied } Please ensure that they are stuck in the relevant place on the registration card and in the players passport book. { if photos not stuck in they will be also be rejected i will not stick them in for you } If you are signing a New Player who hasnt played in the Central Warwickshire League before you will require a form of Original ID either of the following Passport - Medical Card or Birth Certificate with your Players Passport
